class.upload.php is a powerful and mature PHP class to manage uploaded files, and manipulate images in many ways. The script is available under a GPL license.
Is there a way to integrate this into a database? I have profiles that use an upload script and I would like to store the relative link to where the images are being uploaded so they can be accessed by the user they belong to.
also, is there a way to manipulate that scrip to also make thumbnails and move those thumbnails into their designated folders?

You can integrate it in anything you want. You can store the uploaded files in a database if you wish. You can indeed make thumbnails and move these in their designated folders.
